Project Request Form

This form is to be filled out by the Primary Advisor or PI on the project. This is the person the bill will be sent to. Please be sure to include all students and other professors working with you on the project. All fields except additional users are required. You must also click on the acceptance box below that you accept the agreement stated.

Project Name : Please be specific with your project name, do not simply put NSF or DARPA.

Department where funding resides : This is the department that holds the funding for your project. This is not always your home department.

Other Advisors or Professors on the project : (If none, please just write none)
Users of Project : Please list all potential users (students, postdocs, etc).
Additional Users : Use this area for any additional users.
Fund Number: This in an internal UA number associated with your grant or funding source. You must get it from the departmental accountant where the account is kept. Please do NOT make up a number. Your account will not be activated with an invalid number. If you are a user from outside UA please just write "External" in the box.

By checking this box and submitting this request, I acknowledge that I have pre-established funding for the project. I also understand that I am required to submit an annual NSF-style research nugget or highlight of how the CAF instruments assisted in your research. The nuggets will be due in January to the CAF Manager each year for each project that has a CAF account. These nuggets will comprise the research activity report for the CAF. The failure to submit a nugget will result in the loss of use of the Central Analytical Facility. If you have a funded project that contains sensitive information, such as ITAR which cannot be distributed openly, please check the second box.
